Job Posting: R.N. Case Management Director
Location: Medical Center in Englewood, CO
Salary: $146,000 annually
Sign-On Bonus: $10,000
Employment Type: Full-Time
Shift: 10-hour day shift
Position Overview:
We are seeking a highly skilled and experienced R.N. Case Management Director to join our team. This role is responsible for managing and coordinating the activities of the case management department, ensuring compliance with organizational policies and external regulatory agencies. The Director will lead the integration of patient care and discharge planning processes with related hospital departments and external agencies to ensure continuity of care and optimal clinical resource utilization.
The Director will oversee all aspects of daily hospital case management operations and is accountable for achieving established outcomes through actively engaging interdisciplinary teams and external stakeholders. This executive position requires excellent communication and presentation skills, proven motivational capabilities, and a demonstrated track record of delivering results and attaining goals.
Key Responsibilities:
~Direct and evaluate departmental operations, including the case management model, staffing (skill mix and FTEs), use of information technologies, onboarding, and staff competencies to achieve performance and quality objectives.
~Allocate resources to effectively staff the department and meet productivity and quality goals.
~Oversee core case management functions and practices.
~Ensure adherence to care coordination and discharge planning processes, ensuring timeliness, quality, and proper documentation.
~Collaborate with the Facility CFO and Division CM Leader to achieve established goals and expectations.
~Assess and improve the department’s performance by evaluating operational processes, monitoring performance through data analysis, and implementing sustainable performance improvement activities.
~Ensure compliance with policies and SOPs.
~Establish working relationships with key stakeholders, including CMO, CNO, ancillary service leaders, Ethics & Compliance, and Legal.
~Perform other duties as assigned.
~Practice and adhere to the “Code of Conduct” and “Mission and Value Statement.”
Qualifications:
~Bachelor's degree in Nursing (required; LCSW not accepted for this role)
~Master's degree in Nursing, Health Administration, or Business Administration (preferred)
~Minimum of 3 years of overall acute care HCM experience
~Minimum of 2 years in case management leadership in an acute care setting
~Previous HCA experience highly preferred
~ Prior experience as a manager or director at a large facility is essential.
